Purpose: To define methods for maintaining confidentiality and Impartiality.

Scope: Control access to confidential information of the laboratory & maintaining impartiality.

Responsibility:

  1. Quality Manager Laboratory is responsible for maintaining procedure and enforcing the confidentiality and impartiality defined in the procedure.
  2. All persons involved in the testing service are responsible for maintaining the confidentiality of the service recipients.

Procedure:

  • Entrance to the Laboratory's premises is allowed only for employees employed in the Laboratory. Outsiders may enter the Laboratory rooms only with the consent of the Laboratory Manager.
  • Entrance must be labeled “Restricted access”.
  • Do not disclose patient information to any individual unless the patient concerned has consented or the individual is a health professional involved in the care of the patient or required by law or authorized by contractual arrangements to release confidential information, the patient concerned shall be notified of the information released, unless prohibited by law.
  • All information of service providers, such as raw data, test results must be kept confidential.
  • Do not discuss patient information with One patient about another patient, Visitors, News media, Fellow workers.
  • Test results, the data of the service recipients can be accessed by the relevant testing laboratory staff only.
  • Test results are stored and protected against access by external persons by password protective systems in the computers & by giving restricted access of data by authorized personnel as per job description.
  • Privacy & Confidentiality will be maintained during sample collection.
  • All laboratory staff who are trained about this policy must give agreement about confidentiality of service.
  • Laboratory will identify & manage threats to impartiality & confidentiality by the following method.

Step 1: Identification of the risks to impartiality & confidentiality by Direct observation of activity of Laboratory staff & review of Feedback & complaint from users.

Step 2: Analysis and Evaluation of Identified Risks based on impact & frequency of threat

Step 3: Addressing the Risk – Eliminate or Mitigate Identified Risk

  • Minimize in a way that even if the activity of risk is present, we can be assured that it has no significant effect on our laboratory activity results.
  • At this stage is where our corrective actions or opportunities for improvement process or procedure will be used.

Step 4: The laboratory will identify risks to its impartiality on an on-going basis
