                        '' Documentary procedure for Sample receipt''       

Purpose: To ensure accurate and efficient receipt and processing of samples and requests in the laboratory & standardized approach for sample receiving operations in order to ensure high sample integrity and the validity of the analytical results.

Scope: This procedure applies to all laboratory personnel involved in receiving and processing samples and requests.

Responsibility: Laboratory Assistant or Laboratory Technician: Receives and processes samples. Laboratory Manager: Oversees receipt procedure and ensures compliance Laboratory is receiving samples from all the Wards of Sir T Hospital, Bhavnagar & OPD Samples from collection center of Sir T Hospital, Bhavnagar.

Responsibility: Laboratory Technician, Laboratory Assistant, Resident doctors

Procedure: Sample & Requisition form should have all the information as per this document. 7.2.2.b Requisition Procedure

Check for Sample Acceptance & Rejection criteria in all the samples.

BLOOD SAMPLE ACCEPTANCE CRITERIA

  1. Properly filled Requisition forms.
  2. Properly labeled samples.
  3. Details on Requisition form should be matched with sample container details
  4. Proper Vacutainers
  5. Appropriate quantity of sample.
  6. ABG sample & Plasma sample should not be clotted
  7. Spill over the sample should not be there.
  8. Sample will be received within 4 hours of collection.

URINE SAMPLE ACCEPTANCE CRITERIA

  1. Properly filled Requisition forms.
  2. Properly labeled samples.
  3. Details on Requisition form should be matched with sample container details
  4. Appropriate quantity of sample.
  5. Sample containers should not be open, they should be tightly packed.
  6. Sample spillage should not be there.

BLOOD SAMPLE REJECTION CRITERIA

  1. Improperly filled Requisition forms.
  2. Improperly labeled samples.
  3. Details on Requisition form is not matched with sample container details
  4. Improper Vacutainers
  5. Insufficient sample volume.
  6. Clotted sample (Whole blood/Plasma)
  7. Spill over sample.
  8. Hemolyzed sample.
  9. Sample received after 4 hours of collection.
  10. Diluted Sample

URINE SAMPLE REJECTION CRITERIA

  1. Improperly filled Requisition forms.
  2. Improperly labeled samples.
  3. Details on Requisition form is not matched with sample container details
  4. Insufficient sample volume.
  5. Improper sample container
  6. Spill over sample.

Sample acceptance Exceptions

1.Insufficient sample volume.

  • Do not reject the sample.
  • Perform as many examinations as possible as per priority
  • Enter remarks in the report.
  • All pediatric samples with insufficient volume will be received by the laboratory.
  • Priority of testing will be decided by the laboratory incharge or consultant.

2.Hemolyzed sample.

  • Look for grading of hemolyzed samples. From hemolyzed samples Glucose,
  • Tale decision of which parameters can be analyzed from this sample.
  • Enter remarks.

3.Wrong vacutainer- except electrolyte

Registration of all the samples into the LIS.

  • LIS WDI
  • For rejected samples: write the cause of rejection.
  • On registration LIS will generate a Sample ID for the sample.
  • Write following details on received requisition form
  • Sample ID generated by LIS
  • Name of Sample receiver
  • Receiving Date & Time
  • Write Sample ID on received Received Sample.
